At Valley West Animal Hospital, we offer patient forms online so you can complete them in the convenience of your own home or office. Fax us your printed and completed forms or you may bring them during your visit.

We ask that owners with older pets (age 5+) come in the day before their scheduled surgery to submit the completed form while younger animal paperwork can be submitted day of.
**For animals that are 5 years or older, blood work testing is MANDATORY. Animals 5 years or younger have the option to do blood work.
